While our office is not a contracted provider for any insurance company other than Delta Dental, there are certain insurance companies that allow us to obtain benefits that could minimize your out-of-pocket expense. In most cases, we can file the insurance claim forms on your behalf for you to be promptly reimbursed. However, it is the patient’s responsibility to obtain any needed referral or approvals and to know their benefits. Insurance is a contract between the insured and the insurance company and our office is not a party to that contract. Therefore, our office is not responsible for any erroneous information given by the insurance company. Insurance coverage depends on the type of procedure you are going to have as well as the level of coverage your plan offers. Our front office staff is well-versed in different insurance plans, and they will be happy to help you with estimates and other insurance needs.

To expedite the payment and procedure process, we ask that you bring all necessary dental and medical insurance forms to your initial consultation appointment. Also, note that you will be responsible for any outstanding fees charged by our office, regardless of your insurance coverage. Payment is due at the time of service unless other arrangements have been made in advance.
