At Collins Oral & Facial Surgery, we pledge to provide you with the highest level of service in the most efficient manner. Our standards are high, and to keep our patients’ costs to a minimum. we ask that you make PAYMENT IN FULL at the time services are rendered, unless prior arrangements are made and approved. For your convenience, we accept cash, check, Visa, MasterCard, Discover, and American Express.
We also offer CareCredit, which is an outside financial institution that, if you are approved, can allow you to start treatment immediately. With CareCredit, you can enjoy low monthly payments, finance up to 100% of the treatment cost, choose between several payment options — including a no-interest payment plan and extended payment plans — no costs upfront, and no annual fees. You can apply for CareCredit online at their website. We will be glad to answer any questions you may have about either of these financing options.
A deposit is typically required at the time surgery is scheduled. This will be deducted from your final surgical bill. If 48-notice is not given for a cancellation, or if the patient does not show up for surgery, this deposit is forfeited.
While our office does not participate with any insurance company except Delta Dental, we will gladly file the necessary forms in order for you to be promptly reimbursed. However, we are not obligated to withhold our statements or to wait until a settlement has been made before receiving payment for our services. It is the patient’s responsibility to obtain any needed referral or approvals and to know their own benefits. Insurance is a contract between the insured and the insurance company, and our office is not a party to that contract. Therefore, our office is not responsible for any erroneous information given by the insurance company.
An adult/legal guardian must accompany any child under the age of 18 for both the initial evaluation and surgery. The person who brings the child to our office is responsible for full payment regardless of a divorce situation.
Upon arrival, each patient will have a digital x-ray taken. Digital x-rays integrate with our office software and are immediately available for interpretation. Occasionally, additional x-rays or CT scans may be necessary for a treatment plan. This technology allows Dr. Collins and his staff to evaluate each case with a higher level of accuracy than can be obtained through the use of conventional x-rays.
