At Collins Oral & Facial Surgery, we pledge to provide you with the highest level of service in the most efficient manner. Our standards are high so we can keep our patients’ costs to a minimum. We ask that you make PAYMENT IN FULL at the time services are rendered, unless prior arrangements are made and approved. For your convenience, we accept cash, check, Visa, MasterCard, Discover, and American Express.

We also offer CareCredit, an outside financial institution that, if you are approved, can allow you to start treatment immediately. With CareCredit, you can enjoy low monthly payments, finance up to 100% of the treatment cost, choose between several payment options — including no-interest payment plans and extended payment plans — no costs upfront, and no annual fees. You can apply for CareCredit in our office or apply yourself online at their website. We will be glad to answer any questions you may have about CareCredit.

An adult/legal guardian must accompany any child under the age of 18 for both the initial evaluation and surgery. The person who brings the child to our office is responsible for full payment regardless of a divorce situation.

Please note that a deposit is typically required at the time surgery is scheduled. This fee can range anywhere from $150.00 or higher depending on the type and amount of your surgery. Your deposit will be deducted from your final surgical total. If a 48-hour notice is not given for a cancellation, or if a patient does not show up for surgery, this deposit is forfeited.

Typically, your first visit will be a consultation and digital X-ray examination, where Dr. Collins will explain your diagnosis and treatment options. In some cases, additional scans may be necessary to ensure your procedures goes as smoothly as possible. We want you to feel as comfortable as possible with any procedure you elect to undergo, and we will be sure to answer any and all of your questions during this initial consultation.

We ask that you arrive 15 minutes early to your appointment. If you must cancel, we ask that you give us 48 hours’ notice. If you do not give us 48 hours’ notice before a cancellation, you may incur an additional fee.

Please bring the following to your first appointment:

  • Your surgery referral slip and any additional information from your referring doctor
  • A list of all current medications
  • A list of any allergies or other existing medical conditions we should know about
  • Your medical and/or dental insurance information
  • A photo ID

Also, make sure to fill out the new patient paperwork online before the start of your appointment.

Note: All patients under the age of 18 must be accompanied by a parent or guardian.

At Collins Oral & Facial Surgery, we understand you may be nervous about an upcoming procedure. We want to provide you with high-quality, compassionate care and put you at ease in our office, which is why we offer a variety of anesthetic options to make your experience as pleasant and comfortable as possible. The method of sedation used often depends on your preference, the nature of the procedure you will be undergoing, and your medical history. Dr. Collins will work with you to select the best anesthesia option for your treatment.

Your Sedation Options

Oral Sedation: Sometimes a patient may desire to take an oral medication to help them relax or relieve pain during a procedure. This is an option with some procedures, and we occasionally use this technique for minor surgeries.

Local Anesthesia: Local anesthetics are typically applied directly to the area of the mouth where a procedure will be performed. This application numbs the surgical area, and Dr. Collins uses local anesthesia for many different procedures. If you’ve undergone a simple procedure with your general dentist, you’ve probably had local anesthesia before.

Nitrous Oxide: Nitrous oxide, commonly known as laughing gas, is another option that can be used for a patient undergoing a minor procedure, and it can help relax a patient as well as decrease the sensation of discomfort.

IV Sedation/Anesthesia: IV sedation is a very common procedure done in our office, which makes many surgeries essentially painless. IV anesthesia is a great sedation choice if you are nervous about having an upcoming treatment and/or undergoing surgery such as dental implant placement and wisdom teeth removal. Patients typically have no memory of the procedure, and the surgery seems to take no time at all.

General Anesthesia: Some procedures need general anesthesia as performed in a hospital-type environment. We have a hospital-grade anesthesia machine and the ability to do a complete general anesthetic like what is done for a hospital procedure. Very complicated surgeries that take an extensive amount of time can often be candidates for general anesthesia.

Use of a Certified Registered Nurse Anesthetist (CRNA): Dr. Collins has used Clay Rougeau, CRNA, as his anesthesia specialist for many years. Clay has a great bedside manner and tremendous skills in delivering an anesthetic that makes the procedure virtually painless. Although it is not uncommon for an oral surgeon and his staff to administer the anesthesia without a CRNA, Dr. Collins feels that having a CRNA allows for the best care possible for his patients.

Can My Oral Surgeon Administer Anesthesia?

Our surgical team also undergoes rigorous training and certification provided by the American Academy of Oral and Maxillofacial Surgeons as well as Basic and Advanced Cardiac Life Support. This helps ensure that you are receiving the best care possible!

While our office is not a contracted provider for any insurance company other than Delta Dental, there are certain insurance companies that allow us to obtain benefits that could minimize your out-of-pocket expense. In most cases, we can file the insurance claim forms on your behalf for you to be promptly reimbursed. However, it is the patient’s responsibility to obtain any needed referral or approvals and to know their benefits. Insurance is a contract between the insured and the insurance company and our office is not a party to that contract. Therefore, our office is not responsible for any erroneous information given to you by the insurance company. Insurance coverage depends on the type of procedure you are going to have as well as the level of coverage your plan offers. Our front desk staff are well-versed in different insurance plans, and they will be happy to help you with estimates and other insurance needs.

To expedite the payment and procedure process, we ask that you bring all necessary dental and medical insurance forms to your initial consultation appointment. Also, note that you will be responsible for any outstanding fees charged by our office, regardless of your insurance coverage.
